AREA OF COOPERATION
The operation of the cooperative shall be Ilocos Sur and nearby provinces.
MEMBERSHIP
That the common bond of membership of this cooperative is individual, institutional, associational and field of membership shall be opened to all persons who are of legal age, with the capacity to contract or heads of the families. The head is entitled to have four (4 legal dependents, the spouse, and three (3) legitimate children of 21 years old and below. The member, who has no dependent, can designate four (4) beneficiaries properly screened by the management committee and finally approved by the members of the Board of Directors after thorough orientation of the constitution and by- laws and policies of the cooperative.
A member shall not be ALLOWED to withdraw his/her membership from the cooperative unless he desires to sell his/her shares to prospective member who will successfully pass the pre-membership seminar or actual members or to the cooperative itself as per present share value.
MODES/PHASES OF OPERATION According to Share
PHASE 1 - Out Patient Clinic
Share Capital
Two thousand pesos or equivalent to ONE (1) share should have been paid by the member and additional P100 for the membership fee.
Scheme of Payment
The member is encouraged to pay in full, if he/she can but it can also be in instalment basis either weekly or monthly payable within the first four (4) months of the cooperative calendar year. An additional P500 will be paid for additional dependent or beneficiary
Benefits
Free 5 consecutive consultations of family of five for the year. Succeeding consultations shall be charge at the rate of P200 of which 170 shall go to the doctor and P30 to the cooperative.
PHASE 2 - DIAGNOSTICS
Share Capital
Additional share capital of two thousand pesos (P2, 000) or equivalent to TWO (2) shares should have been paid by the member.
Scheme of Payment
The member is encouraged to pay in full, if he/she can but it can also be in instalment basis either weekly or monthly payable within first 4 months of the cooperative calendar year.
Benefits
Same as Phase I (Free 5 consecutive consultations) and 20% discount of all diagnostic examinations.
PHASE 3- MEDICINES
SHARES
Additional share capital of two thousand pesos (P2, 000) or equivalent to three (3) shares should have been paid by the member.
Scheme of Payment
The member is encouraged to pay in full, if he/she can but it can also be in instalment basis either weekly or monthly payable within first 4 months of the cooperative calendar year
Benefits
Same as Phase I (Free 5 consecutive consultations) and Phase 2 (20% discount of all diagnostic examinations) and 3% discount of multinational drugs (branded) and 10% discount of generic medicines.
PHASE 4 - HOSPITALIZATION
After paying six thousand pesos (P6, 000) which is equivalent to 3 shares, the member is eligible to enrol in the Health Trust Fund for hospitalization benefits.
HEALTH TRUST FUND - the amount set aside for future payment of the hospital bills incurred by the participating members and accredited dependents or duly approved beneficiaries confined in the ISCMMGH.
THE QUALIFIER
MEMBER - an actual PHASE 4 regular member qualified for health trust fund at the beginning of the year in which he will be requested to pay yearly annual premium of P2,000.00 for the fund . Failure to pay for the current year means the DISCONTINUANCE or FORFEITURE of HIS/HER HOSPITAL BENEFITS and shares from the dividend from the earnings and savings of the Health Trust Fund for the current year. Reinstatement can be done for the next calendar cooperative year.
DEPENDENTS/BENEFICIARIES - maximum of four (4) legal dependents and four (4) duly approved beneficiaries by the members of the Board of Directors as per screened and recommended by the management committee..
In the absence of duly qualified legal dependents and beneficiaries, a qualified health trust fund member active in the health trust fund for 5 consecutive years shall have the privilege to declare his parents who are 65 and above, brothers and sisters who are 65 and above, still single and with no gainful employment or no other source of income as his beneficiaries if he desires to do so but the exclusion criteria will be strictly followed.
THE FREE HOSPITALIZATION BENEFITS
The family of a qualified member or his beneficiaries when confined at ISCMMGH will avail the Hospital Aid amounting to maximum of P25,000 per year that will be withdrawn from the Health Trust Fund. Hospitalization benefit is broken down to P13,000 for the hospitalization benefit of the member himself and P3,000 each for his/her 4 dependents.
THE FINANCIAL SOURCE
The qualified members are requested to pay a premium of P2, 000.00 per annum. The payment may either be in cash basis or monthly installment to be paid within the first four months of the year (January 1 - April 30).
PHASE 5 - Mobile Clinic
This phase of the operation seeks to serve members and non-member in the far flung areas.
The financial requirement of this phase shall come from the continued capital build-up, savings and donations.
